SUMMER SOCCER CAMP WAIVER AND RELEASE OF LIABILITY

1. I hereby agree to let my child participate in the Florida Soccer Star Camp.
2. I understand that there are inherent risks involved in sports and soccer.
3. I release Florida Soccer Star, Coach Claudio, and all affiliated staff from any liability for injuries or damages.
4. I authorize the staff to obtain medical treatment for my child in case of emergency.
5. I acknowledge that photos or videos of the camp may be used for promotional purposes.
